Summer Camp Forms

 Please note you are not considered registered for camp until all paper work has been completed, submitted and all fees have been paid.

Please read carefully and complete all forms as directed.  Registering on line will enroll you into the email system for proper communication.  Newsletters and important updates are sent electronically throughout the summer.

In order to finalize your application you must print all the forms below and fill them out. These completed forms must be submitted to the Day Care office before your child starts summer camp. Some forms are due immediately at registration and others on/before designated dates.  If you have any questions regarding the forms please contact us.

The following forms have to be in as follows:
Summer Camp trip persmission no later than MAY 25th, 2012.
Summer Camp Lunch Slip no later than MAY 25th 2012.
